Booking Form Question Types

Everything you can ask your attendees before they book. TimelyDo supports 15 question types. From simple text fields to file uploads, star ratings, and handwritten signatures.

What are booking form questions?

Booking form questions are custom fields your attendees fill in before they confirm a booking. You build a reusable library of questions at /questions, then assign whichever ones you need to each individual event.

TimelyDo supports 15 distinct question types. More than any other scheduling platform. Every question can be marked required or optional, and only active questions can be assigned to events.

How questions work

Questions live in a central library and are reused across events. The workflow is two steps:

Create in the library
Go to Form Questions in the sidebar. Create a question, choose its type, mark it active.

Assign to an event
Open any event in the editor and add the questions you want shown on that event's booking form.
Tip: Questions in Draft state are hidden from the event editor. Toggle a question to Active before trying to assign it.
Default values: Most question types support a default value. When set, the field arrives pre-filled on the booking form. The booker can still change it. For Hidden fields the default is stored silently with every booking and is never shown to the booker.

Text Fields

One Line Text

Short single line answer
one_line

A single line text input for short, focused answers. The most versatile question type. Use it whenever you need a brief, open-ended response that fits on one line.

When to use
  • Company or organisation name
  • Job title or role
  • Referral source ("How did you hear about us?")
  • Project name or reference code

Multi Line Text

Long form paragraph answer

A resizable textarea for longer, detailed responses. Ideal when attendees need space to explain context, share background, or provide detailed notes.

Number

Numeric input with validation
number

A numeric input that only accepts numbers. You can set an optional minimum and maximum value to constrain the range. Non-numeric characters are rejected by the form.

When to use
  • Number of attendees or guests
  • Age or years of experience
  • Budget (approximate figure)
  • Number of sessions needed

Contact Fields

Phone Number

International phone input with country selector
phone_number

A dedicated phone number input with a country code selector. Attendees pick their country flag and dial code from a dropdown, then enter their local number. Useful when you need to call or SMS attendees before or after a session.

When to use
  • Coaching or consulting calls
  • Reminder or confirmation via SMS
  • In person appointments requiring a callback number
  • Any service where a phone follow up may be needed

Email

Email address with format validation
email

An email address field with built in format validation. The browser will reject obviously invalid entries (missing @ sign, etc.) before the form is submitted. Use this when you need a second email address in addition to the one used to make the booking.

When to use
  • Collecting a team lead or coordinator email
  • "Send the report to" address
  • Billing email separate from the booking email

Note: Every booking already captures the attendee's primary email. Only add this field when a second, distinct email address is genuinely needed.

URL

Website link with URL validation
url

A link input that validates the entry is a properly formatted URL. Useful for collecting portfolio links, social profiles, websites, or any web address you need to reference before or after the session.

  • Portfolio or work samples
  • LinkedIn or social profile
  • Company website
  • A specific document or page to review

Choice Fields

Radio Buttons

Single choice, all options visible
radio_button

A single-select list where all options are displayed at once as radio buttons. The attendee can pick only one. Great for short, clearly distinct options where you want the full list visible without any interaction.

When to use
  • Meeting format (Video / Phone / In person)
  • Experience level (Beginner / Intermediate / Advanced)
  • Preferred time of day
  • Gender (for health forms)

Checkbox

Multiple selections allowed

A multi-select list where the attendee can tick as many options as they like. Unlike Radio and Dropdown, Checkbox allows zero, one, or many selections from the list.

  • Pain points or challenges
  • Availability windows (select all that apply)

Yes / No

Simple binary choice
boolean

A minimal binary selector. Just Yes or No. Cleaner than a Dropdown or Radio when the answer is strictly binary and there is no ambiguity between the two options.

When to use
  • "Are you an existing client?"
  • "Do you have a specific date in mind?"
  • "Have you completed the intake form?"
  • Any simple agree/disagree question

Date

Date, date range, time, or date and time picker

A flexible temporal question type that supports four formats. Choose the format when creating the question. The booking form shows the matching picker and stores the answer in a consistent format.

Worth knowing: a date question here is separate from the meeting slot itself, so you can ask about a project deadline or a preferred start date. All four temporal formats live in a single question type.

Rating

5 star or 0 to 10 NPS scale
rating

A visual rating scale with two modes: a classic 5 star rating, or a 0 to 10 NPS (Net Promoter Score) scale. Use it to collect a satisfaction score, intent signal, or confidence level before the session begins.

When to use
  • Pre session confidence or readiness score
  • NPS before a consulting session
  • "How satisfied were you with your last provider?"
  • Pain level (1 to 10) for health appointments

Worth knowing: the rating question is built in, so a satisfaction score or an NPS style answer can be collected on the booking form itself rather than through a separate survey tool.

File Upload

Accepts documents and images

Lets the attendee attach a file to their booking. You can restrict which file types are accepted and set a maximum file size (up to 5 MB). Accepted formats: PDF, Word documents (doc, docx), images (jpg, jpeg, png, gif, webp), CSV, and plain text.

When to use
  • CV or resume before an interview
  • Creative brief for design sessions
  • Medical record or referral letter
  • Tax documents for an accountant call
  • Property photos for a valuation

File size limit: Individual uploads are capped at 5 MB. If attendees need to share larger files, ask them to provide a link via the URL question type instead.

Signature

A canvas-based signature field where the attendee draws their handwritten signature using a mouse, trackpad, or finger on a touchscreen. You can add optional declaration text above the signature box, such as a waiver statement or terms acknowledgement. The signature is captured as an image and stored with the booking.

When to use
  • Health and fitness liability waivers
  • Photography or creative consent forms
  • Legal or coaching terms of service
  • GDPR or data processing acknowledgements
  • Any booking that requires a formal sign-off

Worth knowing: the signature field is built in. If your work requires a signed waiver or consent before every booking, you can capture it on the booking form rather than sending the attendee through a separate signing tool.

Hidden

Not visible to the attendee
hidden

A hidden field is never shown on the booking form. It stores a fixed default value that gets saved alongside the booking behind the scenes. Used for internal tagging, source tracking, or passing context that the attendee should not see or edit.

When to use
  • Tag bookings by campaign or channel (e.g. "instagram-promo")
  • Record which event template was used
  • Store an internal category for CRM export
  • Track A/B test variant
Configuration options
Default value (always stored)

No required/optional toggle. The attendee never sees this field. The default is stored with every booking and can be overridden per link by appending ?field_name=value to your booking URL.

Internal use only: Hidden fields appear in your booking records and CSV exports but are never displayed to the person making the booking. You can use different hidden field values across multiple events to distinguish where bookings originate.
